Biggest Problems in Global Health

So this thread has been relatively dead, so I'm going to try and spark it :)
What do people think is the most important issue regarding global health? Malaria? HIV/AIDS? TB? Maternity health?
I'm not sure what I think it is the most important, but HIV/AIDS is definitely way up there...
